If you are looking for a family doctor, you can now apply online to find one. Persons already registered on a waiting list will retain their priority.
Replacement of a lost, stolen or deteriorated Health Insurance Card
You can request the replacement of a lost, stolen or deteriorated card. The new card will display the same photo, signature and expiry date as on the card being replaced.
Medical certificate
Form to be filled out by a physician so that you may be exempted from providing your photo or your signature on your Health Insurance Card.
Sworn statement
A person is said to be making a sworn statement (also referred to as a "solemn affirmation"), when he/she makes one or more assertions before a commissioner for oaths. This is necessary, for instance, when someone is unable to provide documented proof.

Do you consider that you were wrongfully billed for a medical, dental, optometric or pharmaceutical service (other than drugs)? Effective December 7, 2015, you have 5 years from the date of payment of services to file a request for reimbursement.
Healthcare received outside Québec
Healthcare received outside Québec is covered under certain conditions. You have 1 year to request a reimbursement for professional services received and 3 years in the case of hospital services.
Change of address
You can submit your change of address on the Internet by using the Service québécois de changement d'adresse. This will enable you to give your new address to 6 departments and agencies at the same time.
Prescription drug insurance
Registration and deregistration
To benefit from the Public Prescription Drug Insurance Plan, it is necessary to register. And when you become eligible for a private plan, you must deregister from the public plan, which you may do online (if your family situation and your age allow you do use this service).
Checking your student status
When you send the Régie the information required to confirm your status of student covered by the Public Prescription Drug Insurance Plan, make sure to have your Health Insurance Card handy, as well as the file number that we mailed you.
Aid programs
External breastforms
The External Breastforms Program is intended for all women who have undergone a total or radical mastectomy, and for women age 14 and over who have a total absence of breast formation, medically diagnosed as aplasia.
Ostomy
The Ostomy Appliances Program is intended for persons who have undergone a permanent colostomy, ileostomy or urostomy of which the permanent nature is attested to by a medical certificate.
Compression garments for the treatment of lymphedema
The Compression Garments Program for the Treatment of Lymphedema is directed at anyone who has primary or secondary lymphedema.
Accommodation in a public facility
Use the simulation tool to obtain an estimate of the contribution amount requested of a person accommodated in a Québec health facility. Please note that the staff at the facility is responsible for registering adults.

To officially record your consent to organ and tissue donation, you have to fill out the form intended for this purpose.
Third party liability
By law, as soon as an insurer finds out that a client is responsible for causing a physical injury and that the Régie will or may have to pay for covered services as a result, it must notify the Régie in writing.

Power of Attorney
The Power of Attorney is a document whereby a person designates someone else to act on his/her behalf.
History of health care received
This form is intended for insurance-sector workers who received prior authorization, while the person concerned was alive, to access information held by the Régie about him/her for the purpose of processing, administering, or settling a life-insurance policy.
